An internal investigation has confirmed new security breaches that could have opened the door to an assassination attempt on Donald Trump.
It has emerged that agents never instructed local police to guard the roof of the building used by the shooter to assassinate Trump.
Two high-ranking government officials spoke to The Washington Post about this.
The investigation found that agents from the Secret Service and the Pittsburgh field office used a defensive strategy to prevent the shooter from clearly seeing Trump.
However, inspectors who arrived in Butler on the day of the rally found that cranes, trucks and flags were not properly unfurled.
So the shooter managed to climb to the roof of the building and wound Trump in the ear.
The investigation also found glaring weaknesses in the Secret Service's event communications system.
The Secret Service radio room had no way to receive real-time alerts from the police.
The report also found that Trump's security measures were not adequately tightened.
